Re: Dispute of inaccurate information on my credit report

To whom it may concern:

I am writing under the Fair Credit Reporting Act (15 U.S.C. § 1681i) to dispute inaccurate information on my credit report.

Item in dispute: [Creditor name] and [collector name] — accounts ending [xxxx] and [xxxx]

Why it is inaccurate: These entries report the same debt more than once, which inaccurately inflates my balances. The original creditor entry should show a zero balance if the debt was sold.

What I am asking: Please investigate and correct or delete the duplicate entry.

Please send me the results of your investigation in writing, along with an updated copy of my credit report.

Sincerely,
[Signature]
[Your printed name]

Enclosures:
- Statements or letters showing both entries relate to one debt
- Collection notices that name the original creditor
